• Human remains: To export remains. the following documents are required

- Death certificate or Coroner's sertificate permitting disposal
- Embalming certificate issued by the funeral director;
- Packing certificate issued by the funeral director;
- Infectious diseases certificate issued by the Department of Health certifying that there were no inffectious diseases in plague proportions in the State at the time of the death;
- Export authorization issued by the Department of Health/Human services;
